To receive your rebate, you'll need to submit a copy of your original itemized receipt from your doctor showing proof of treatment, plus the sticker(s) from the EVOLENCE^® box(es) that was used for your treatment. You'll also need to register on this website, complete and submit an EVOLENCE^® Rebate Request Form.

To thank you for your EVOLENCE^® treatment performed between January 1, 2009 and December 31, 2009 you can receive a $50 rebate by mail for each syringe used for your EVOLENCE^® treatment (up to three 1mL syringes) with a qualifying rebate request form. Limit to two rebate submissions per patient per calendar year. Patient must be 18 years or older to qualify.
