To receive your rebate, you'll need to submit a copy of your original itemized receipt from your doctor showing proof of treatment, plus the sticker(s) from the EVOLENCE® box(es) that was used for your treatment. You'll also need to register on this website, complete and submit an EVOLENCE® Rebate Request Form.
To thank you for your EVOLENCE® treatment performed between January 1, 2009 and December 31, 2009 you can receive a $50 rebate by mail for each syringe used for your EVOLENCE® treatment (up to 3 syringes) with a qualifying rebate request form. Limit two rebate submissions per patient per calendar year. Patient must be 18 years or older to qualify.
